Evaluation of your DJ / KJ

Most DJ or KJ companies will send out an evaluation form shortly after your event. If they do not, or you have other guests that wish to make comments known about the service you hired, use this PDF form, fill it out and send it to your service provider or to our association (which we will keep a copy on file and send a copy to the DJ or KJ Service for you).
